چکیده

Background Axial Spondyloarthritis is an umbrella term including radiographic (r-axSpA) and non-radiographic (nr-axSpA) forms which has been initially suggested by Assessment of SpondyloArthritis International Society (ASAS) as a different stages the disease. Within time, studies have tried to answer whether these two entities represent distinct features, debate still ongoing. Objectives The objective this study define similarities differences r-AxSpA nr-AxSpA patients using large national registry data. Methods TReasure national, multicentre, longitudinal, observational database, initiated in Turkey 2017 [1]. This database included with inflammatory arthritis, rheumatoid axial-and peripheral SpA at time first introduction biological treatment. At analysis consisted 7015 patients. All registered fulfill 2009 ASAS classification criteria. For analysis, were categorized into demographic clinical characteristics determined. Results There 4629 (86.7%) r-AxSpA, 707 (13.3%) Demographic ; group older (45 (16) vs 41 (14), p<0.001) male predominant (59.6% 46.0%, p<0.001), longer disease duration bDMARD initiation (3.0 (6.9) 1.1 (3.4) years, more smoker (61.4% 50.4%, p<0.001). clinic/genetic had less arthritis (7.7% 27.5%, heel pain (11.1% 40.8%, dactylitis (3.4% 8.1%, psoriasis (10.2% 14.5%, family history (28.2% IBD (4.5% 7.4%, p=0.001) HLA-B27 positivity (58.2% 51.4%, similar uveitis rate (13.2% 14.8%). Outcome measures (bDMARD baseline) differences; worse functionality quality life measured BASFI (5.0 (3.0) 4.0 (3.9), HAQ-DI 0.63 (0.40) 0.55 (0.65), p=0.029) respectively. Worse metrology BASMI 3.2 (2.6) 1.7 (2.1), high activity ASDAS-CRP (3.64 (1.28) 3.24 (1.33), higher CRP (12 (21.8) 7.3 (15.0) was observed. imaging `any syndesmophytes `(26.6% 5.4%, hip involvement (24.6% 4.5%, prosthesis (2.3% 0.3%, MRI structural lesions (63.6% 37.3%, active (74.6% 87.0%, No significant difference observed physician decision on b-DMARD choice baseline (p=0.673). Conclusion A proportion who r-SpA. Although are seen continuum each other, they terms demographic, clinical, genetic, functional, metrology, characteristics. On other hand, there no treatment choices stage initiation. Reference [1]Kalyoncu U, et al. Turk J Med Sci. 2018 Aug 16;48(4):856-861. Acknowledgements: NIL. Disclosure Interests None Declared.

Radiographic axial spondyloarthritis versus ankylosing spondylitis.

Sirs, Braun et al. in their letter claim that in clinical practice there is no need to differentiate between a diagnosis of ‘radiographic axial spondylaorthritis (axSpA)’ and ‘non-radiographic axSpA (nr-axSpA) (1). Implicit in their reasoning is the view that nr-axSpA and AS represent one single entity, even though differences between the two entities have been reported, including gender, HLA-B...

Effectiveness of Adalimumab in Non-radiographic Axial Spondyloarthritis

The primary aim of the study was to evaluate the long-term effectiveness of adalimumab (ADA) in a cohort of non-radiographic axial spondyloarthritis (nr-axSpA), and the secondary aims were to identify predictive factors of response and evaluate radiological progression. We evaluated 37 patients (male/female: 12/25; mean age 49 ± 14; mean disease duration: 6.3 ± 5.8) with active nr-axSpA (Assessm...

Extra-articular manifestations and burden of disease in patients with radiographic and non-radiographic axial spondyloarthritis.

OBJECTIVE Although the prevalence of peripheral and extra-articular disease in ankylosing spondylitis (AS) has been assessed in many studies, data on non-radiographic axial spondyloarthritis (nr-axSpA) is scanty. The aim of this study was first, to compare radiographic-axSpA/AS (r-axSpA/AS) and nr-axSpA concerning peripheral arthritis and extra-articular manifestations (EAMs), and second, to as...

OBJECTIVE To assess whether factors such as inflammation by laboratory tests and MRI differ between early axial SpA with and without radiographic lesions. METHODS Cross-sectional analysis of baseline data from Devenir des Spondylarthropathies Indifferenciées Récentes (DESIR) cohort patients having recent-onset inflammatory back pain and meeting Assessment of SpondyloArthritis international So...

The Nonradiographic Axial Spondyloarthritis, the Radiographic Axial Spondyloarthritis, and Ankylosing Spondylitis: The Tangled Skein of Rheumatology

Since 1984 the diagnosis of ankylosing spondylitis (AS) has been based upon the modified New York (mNY) criteria with mandatory presence of radiographic sacroiliitis, without which the diagnosis is not tenable. However, it may take years or decades for radiographic sacroiliitis to develop delaying the diagnosis for long periods. It did not matter in the past because no effective treatment was a...
